Directory of AIDS Drug Assistance Programs

Connecticut

The AIDS Drug Assistance Program ( ADAP ) helps ensure that people living with HIV and AIDS who are uninsured and under-insured have access to medication.

Eligibility Criteria: 

In order to be eligible for CADAP, you must:

  • Be a Connecticut resident
  • Be diagnosed with HIV
  • Have a total household income below 400% of the Federal Poverty Level (FPL)
  • Apply for any health insurance coverage that you may be eligible for, if you do not already have coverage.
Application Instructions: 

Please complete Sections I, II, III, and V of the CADAP application form. Your medical provider must complete Section IV. Mail the completed application (all sections) to the CADAP mailing address below:

Effective November 1,2015, the CADAP Program will be requiring income and residency verification for first time and renewal applications.
Hepatitis C (HCV) Drug Therapy:

State covers some form of HCV drug therapies as part of their approved drug formularies: Yes
