The AIDS Drug Assistance Program (ADAP) helps ensure that people living with HIV and AIDS who are uninsured and under-insured have access to medication.
In the State of New York, to be eligible for ADAP enrollment, an individual must meet the following criteria:
A single application form can be used to enroll in ADAP, ADAP Plus, the HIV Home Care Program and PrEP-AP. An additional application is required for APIC. Applicants apply to the program, providing proof of residency and income. A Medical Application signed by a licensed medical professional is required, verifying HIV status.
When you are approved, you will get an Eligibility Card and instructions on how to use it. You must present this card and prescription at a participating pharmacy to receive covered medications at no charge. Show your card to participating health care providers to receive covered medical services at no charge. If you need them, you will receive home care services from an enrolled home health care agency at no charge ($30,000 maximum life-time benefit).
