To be eligible for the program, a person must be a Rhode Island resident, must have a documented HIV diagnosis, and have an income less than 4 times (400%) the Federal Poverty Level (FPL). For example, for a family of one, 400% of the poverty level for 2014 is $46,680. For more information about the poverty guidelines, go to U.S. Department of Health and Human Services' Poverty Guidelines.
To apply for ADAP, the health care provider and the patient must complete an ADAP Application Form and fax it to the Executive Office of Health and Human Services. EOHHS staff will determine the patient's eligibility for the program and contact the patient and their health care provider to inform him/her they are enrolled in the program. For most people, enrollment into ADAP takes less than 48 hours.
