Directory of AIDS Drug Assistance Programs

Minnesota

The AIDS Drug Assistance Program ( ADAP ) helps ensure that people living with HIV and AIDS who are uninsured and under-insured have access to medication.

Eligibility Criteria: 

To be eligible for the AIDS Drug Assistance Program (ADAP) in Minnesota, a person must:

  • Be HIV positive
  • Be uninsured or insured with a drug co-pay
  • Have income below 400 percent of the Federal Poverty Guidelines.

You cannot be on Medical Assistance, Medical Assistance for Employed Persons with Disabilities or MinnesotaCare.

Application Instructions: 

Complete the New Applicant form along with the Ryan White Annual Certification Form and Ryan White Healthcare Provider Request form. (see application for additional form links)

Submit the forms and all required documentation to Minnesota Dept. of Human Services, P.O. Box 64972, St. Paul, MN 55164-0972 or by secure fax to 651-431-7414.
